Preterm birth: temporal trends and socioeconomic inequalities.

چکیده
Socioeconomic inequalities in perinatal health have been consistently observed across many highand low-middle income countries. Quantifying and monitoring socioeconomic inequalities in health is an important first step toward reducing health inequity and improving population health. Brazil has long been ranked among the countries with the largest inequalities in both socioeconomic position and health. In this issue of the Jornal de Pediatria, Sadovsky et al. report income inequalities in preterm birth (PTB, <37 completed weeks of gestation) in the city of Pelotas over a 30-year period. The authors estimated the slope index of inequality (SII) and the relative index of inequality (RII) of income in PTB rates among almost all births in Pelotas in 1982, 1993, 2004, and 2011. Relative and absolute health differences between groups provide different and complementary information, which can lead to different conclusions, particularly when the focus is monitoring changes in inequality over a prolonged time period. Proportions of a population within any socioeconomic group -e.g., individuals with a university education -inevitably change over time and may differ by

No improvement in socioeconomic inequalities in birthweight and preterm birth over four decades: a population-based cohort study
BACKGROUND Birthweight and gestational age are associated with socioeconomic deprivation, but the evidence in relation to temporal changes in these associations is sparse. We investigated changes in the associations between socioeconomic status (SES) and birthweight and gestational age in Newcastle upon Tyne, North of England, during 1961-2000. METHODS We used population-based data from hospi...
متن کاملSocioeconomic inequalities in survival in neonates Access to good intensive care blunts the effects of deprivation
In the linked prospective cohort study, Smith and colleagues assessed socioeconomic inequalities in survival and neonatal care provision among very preterm infants.1 A cohort of 7449 infants born during a 10 year period (1998-2007) at between 22+0 and 32+6 weeks’ gestation were followed up until discharge from neonatal care. What do the findings suggest about equity of neonatal care provision? ...
متن کاملExplaining educational inequalities in preterm birth: the generation r study.
BACKGROUND Although a low socioeconomic status has consistently been associated with an increased risk of preterm birth, little is known about the pathways through which socioeconomic disadvantage influences preterm birth. AIM To examine mechanisms that might underlie the association between the educational level of pregnant women as an indicator of socioeconomic status, and preterm birth. ...
متن کاملTemporal trends in neonatal outcomes following iatrogenic preterm delivery
BACKGROUND Preterm birth rates have increased substantially in the recent years mostly due to obstetric intervention. We studied the effects of increasing iatrogenic preterm birth on temporal trends in perinatal mortality and serious neonatal morbidity in the United States. METHODS We used data on singleton and twin births in the United States, 1995-2005 (n = 36,399,333), to examine trends in...
متن کاملTemporal trends of preterm birth in Shenzhen, China: a retrospective study
BACKGROUND Preterm birth is the leading cause of child mortality under 5 years of age. Temporal trends in preterm birth rates are highly heterogeneous among countries and little information exists for China. To address this data gap, we investigated annual changes in preterm birth incidence rate and explored potential determinants of these changes in Shenzhen, China. METHODS A total of 1.4 mi...
